Obtain A Repeat Prescription?
Some of you will be taking regular medicines. Repeat prescriptions may be requested without seeing a doctor.
Please give us:
24 hours' notice for a chemist's prescription
Or
48 hours' notice for a dispensed prescription
*PLEASE NOTE no orders will be taken by telephone except in an emergency.
Return your re-order slip which is attached to all dispensed prescriptions by hand or post to the surgery.

We are allowed to dispense to you if you live more than one mile from a chemist (ie, you can collect your prescriptions from the surgery).
If you live less than a mile away from a chemist - Lloyds and Day Lewis Chemists of Coleford call to collect prescriptions each day.
If you would like to partake of this service, please indicate when ordering. We recommend regular supervision; ask if you have not seen a doctor recently.
Prescription Charges
Prescritions are charged per item for patients not on the exempt list.
The prescription charge is a contribution to the NHS and is NOT a payment to the practice. It bears no relatonship to the cost of the medicine.
Pre-payment certificates can be purchased at the reception for three months or one year.
Prescriptions are free for patients:
Under 16
Aged 16, 17 or 18 and in full-time education
Aged 60 years or over
Named on a current:
HC2 certificate
Working tax credit exemption or child tax credit exemption
Or who
Hold a current:
Maternity exemption certificate
Medical exemption certificate
War pension exemption
